There is a $50 deposit collected at the time of scheduling to hold your appointment, which will be deducted from your appointment cost when we meet.   If you cancel your appointment at least 48 hours prior to your scheduled time, you will be refunded the deposit.  
Important:  After you pay the deposit you will receive a confirmation email with a link that says "Download".  That is the form you need to complete all the forms prior to our appointment, which I need at least 48 hrs before our appt.  
DO NOT SELECT SHIPPING, SELECT LOCAL PICKUP OR YOU WILL BE CHARGED A SHIPPING FEE.  THE SYSTEM DOES NOT ALLOW ME TO CREATE A PRODUCT WITHOUT A SHIPPING OPTION.
